AXA Global Healthcare vs Bupa Global is a comparison of two fixed-tier international health plans, both with medical evacuation on every tier. AXA sells five tiers with annual limits from US$400,000 to US$8,000,000. Bupa sells five, from US$1,700,000 to unlimited. The terms that separate them are underwriting and age: AXA publishes a moratorium and an age 80 threshold for it, while Bupa's individual applications are fully medically underwritten.

We read both insurers' own documents on 29 September 2026 and set 17 terms side by side. There is no overall winner: your age, medical history and where you live decide which terms matter.

ExpatMedicare compares plans from 25+ international insurers. About us.

Which Bupa insurer covers you depends on where you live; ask before buying. The Bupa figures on this page come from Bupa Insurance Limited's April 2026 brochure. Bupa's target market statement for that product lists customers living outside the UK, Channel Islands and Gibraltar among those it is not designed for, and Bupa does not publish which insurer and documents apply elsewhere.

Who insures you with AXA and with Bupa?

AXA Global Healthcare is a brand with three insurers behind it. Its Standard plan documents name AXA XL Insurance Company UK Limited and AXA PPP healthcare Limited, both regulated in the UK by the PRA and FCA, and AXA Insurance dac, regulated by the Central Bank of Ireland (AXA XL, AXA PPP, AXA Insurance dac). AXA does not publish which applies to whom, so ask.

Bupa Global is, in Bupa's words on its legal notices page, "the international health insurance division of Bupa". The plans compared here are issued by Bupa Insurance Limited (UK). EEA policies come from Bupa Global DAC (Ireland), and Hong Kong plans are a separate product from Bupa (Asia) Limited, per Bupa's Hong Kong application form. The insurer named on your policy decides which regulator oversees it and where a complaint goes.

How do the annual limits compare?

Bupa Global reaches higher at the top, because Ultimate has no annual limit. AXA Global Healthcare starts lower, with Foundation at US$400,000.

Sources: AXA summary of benefits, p. 3; Bupa April 2026 brochure, p. 12. Rows are ordered by limit. Bupa's Major Medical has a higher limit than Select or Premier but no outpatient cover, so compare the tier you would buy, not the tier in the same row.

What is built into each tier?

AXA Global Healthcare and Bupa Global both include medical evacuation on every tier. AXA's individual sales brochure says "All plans include emergency evacuation and repatriation as standard", outside "your overall benefit allowance" per its evacuation page. Bupa's brochure lists evacuation as "Paid in full" on all five tiers.

Outpatient cover is where the tiers split:

  • AXA Foundation and Standard: outpatient is an optional upgrade, with an allowance of US$1,600 or US$4,000 a year.
  • AXA Comprehensive and Prestige: built in, up to US$5,600 and US$16,000 a year.
  • AXA Prestige Plus: consultations and tests covered up to the overall plan limit.
  • Bupa Major Medical: no outpatient cover.
  • Bupa Select, Premier and Elite: built in, up to £7,500, £15,000 and £50,000 a year.
  • Bupa Ultimate: unlimited.

Sources: AXA summary of benefits pp. 5 to 6; Bupa brochure p. 13. Dental waits: Bupa publishes 6 months (12 for orthodontics); AXA publishes none.

When does maternity cover start?

AXA Global Healthcare and Bupa Global both wait 18 months, and both keep maternity on their top two tiers only.

  • Routine pregnancy and birth: AXA up to US$16,000 on Prestige and US$19,200 on Prestige Plus; Bupa up to £10,000 for a normal birth on Elite and paid in full on Ultimate.

Sources: AXA's maternity page ("not available as an added extra"); Bupa brochure p. 16. If you plan to move up to a tier with maternity later, ask in writing whether time on your current tier counts toward the 18 months.

How do they underwrite pre-existing conditions?

AXA Global Healthcare publishes three routes. Bupa Global publishes one.

AXA Global Healthcare, per its retiree cover page:

  • Fully underwritten: you disclose your medical history and AXA tells you which exclusions apply.
  • Continuing medical exclusions: switching from another insurer, "we may continue applying these exclusions to your new plan with us".
  • Moratorium: conditions you had in the five years before joining are excluded "until you have been a member for two years in a row and you have had a period of two years trouble-free from that condition".

Bupa Global: pre-existing conditions are excluded unless a medical loading is agreed, per its brochure. Bupa's UK and Channel Islands and EEA application forms both state that "the policy you are applying for is fully medical underwritten". No moratorium appears in any Bupa document we read.

Is there an age limit to join?

AXA Global Healthcare sets its threshold at 80. Its retiree cover page says you "must be under the age of 80 years old at the start of cover" for moratorium or switch terms. From 80, only full medical underwriting or a plan transfer, and "there will be some exclusions".

Bupa Global's target market statement (May 2026) says "There is no upper age limit" for Bupa Global Health Plans issued by Bupa Insurance Limited. That document is written for brokers, and Bupa does not publish an entry age for its other insurers, so confirm it for the insurer that would issue your policy.

At renewal, AXA says claims will not raise your premium, though age and inflation can. Bupa marks automatic renewal and no cancellation over claims history as Yes on all five tiers (brochure).

How do they handle treatment in the United States?

AXA Global Healthcare and Bupa Global both include the USA only on their worldwide option. Choose worldwide excluding the USA and each keeps some emergency cover there:

  • AXA: emergency inpatient and day-patient treatment on every tier except Foundation: 6 weeks (US$16,000 Standard, US$24,000 Comprehensive) or 10 weeks (US$32,000 Prestige, US$48,000 Prestige Plus, plus US$3,200 outpatient), per page 8 of its summary of benefits.
  • Bupa: 28 days of emergency US cover on the worldwide without US option, per page 12 of its brochure. Bupa is an independent licensee of the Blue Cross Blue Shield Association.

Bupa's Select, Premier and Elite also offer a Europe (including UK) area. The AXA documents we read offer worldwide options only.

Can you cancel, and what does paying monthly cost?

AXA Global Healthcare gives 14 days from receiving your membership pack, refunded if no claims were paid. After that, its Standard plan documents (AXA XL, AXA PPP, AXA Insurance dac) let monthly and quarterly payers stop at the next payment date; annual payers get a pro-rata refund for whole months left, less a US$70 (£50) administration fee and that year's claims.

Bupa Global publishes cancellation terms for Ultimate only. Its Ultimate membership guide refunds the year's premium if you cancel within 30 days with no claims in that time; otherwise cancellation takes effect 14 days after you ask. For other tiers, ask for the terms in writing.

On payment frequency, AXA's plans page says "Paying annually will save you 5% on your total cost". Bupa does not publish a figure.

How much do AXA and Bupa cost?

What you pay depends on your age, area of cover, deductible, tier and medical history, so we do not publish price comparisons between insurers. Our expat health insurance cost guide explains what moves a premium and where the published evidence sits.
